What is male thoracic surgery?

It refers to a group of procedures designed to flatten, define, and masculinize the chest, creating a firm and proportionate thoracic contour.

Dr. César López Romero has extensive experience in male chest surgery and customizes each technique according to the patient’s anatomy, needs, and goals.

Within this area, two main types of procedures are primarily performed:

Gynecomastia Surgery:
Gynecomastia is the enlargement of the male chest caused by excess glandular tissue, localized fat, or a combination of both, and it does not always improve with diet or exercise. This procedure removes the excess breast tissue and associated fat, achieving a flatter, firmer, and more athletic chest that is harmonious with the rest of the torso.

Subcutaneous (Double) Mastectomy:
Subcutaneous mastectomy is most commonly performed in transgender men or cisgender men with more pronounced breast development. The goal is to create a smooth, symmetrical, and masculine chest contour, preserving the nipple and areola whenever possible while respecting the body’s natural proportions.

In both cases, the result is a torso that reflects male proportions and eliminates the psychological and physical discomfort caused by excess breast tissue.

Is this procedure right for you?

Gynecomastia surgery is ideal for men with localized breast tissue or excess chest fat that doesn’t respond to weight loss. Mastectomy may be recommended for those with more pronounced breast development or for transgender men seeking chest masculinization surgery as part of their gender-affirming care.

Types of Male Thoracic Surgery: Gynecomastia and Mastectomy

Gynecomastia surgery

It is usually performed in cases of mild to moderate excess glandular tissue or fat in the chest.

The procedure removes the excess tissue, preserves the nipple, and aims to minimize scarring, which is typically placed around the areola or within the natural crease of the armpit, achieving a flatter and more masculine contour.

Double mastectomy

Surgery removes all of the mammary gland tissue, and while it is often possible to preserve the nipple, it may be necessary to reposition it or make minor revisions.

The process and recovery

Both the surgical and recovery processes follow a structured and safe plan, personally guided by Dr. César López Romero and his team.

Will the results look natural?

Absolutely. Dr. López's goal is not just to be flat, but to achieve a proportionate, firm, masculine-looking chest that integrates with the overall structure of the torso.

Are the scars visible?

The incisions are strategically placed along natural edges, such as the areola or chest crease, and usually disappear over time with proper care.

Do I need to lose weight before surgery?

While not required, achieving a stable and healthy weight improves results and reduces the chance of needing skin removal or revision.

Will the chest remain flat over time?

Yes, Once removed, glandular tissue does not return. Maintaining a stable weight and hormone levels ensures that the result stays consistent.

Can gynecomastia surgery be combined with other aesthetic procedures?

Yes. Depending on your goals, gynecomastia surgery can be combined with abdominal liposuction or body contouring procedures to achieve a more harmonious and proportionate result.Dr. César López Romero will evaluate each case individually and design a comprehensive, personalized treatment plan.
